Card Access CMCA at the Harry Perkins Institute of Medical Research

All users (including UWA staff and students) accessing CMCA@ Perkins facilities in the Harry Perkins Institute of Medical Research must apply to CMCA for a swipe card access to the building.

A request form for card access to the Harry Perkins Institute of Medical Research (HPIMR) building can be collected from the concierge desk on the ground floor of HPIMR or downloaded from the link below.

This should be completed (entering CMCA in the department field). CMCA staff will specify on the form which CMCA facilities (e.g. PC2, MRI suite etc) in Perkins will be accessible to the user using appropriate building zone references. Standard access will be limited to office hours.

The completed form must be signed by the user and approved by the relevant technique group leader. This form will be forwarded by CMCA staff to HPIMR management for sign off, who will forward it to the concierge. A copy of the completed form will also be sent to CMCA administration.

The new card may be collected within 48 hours from the concierge on the ground floor of HPMRI. A photograph of the applicant will be taken for printing on the card and a basic building induction carried out.

If a change in access area within CMCA is required at a later date, a new form must be filled out specifying the details and signed by the relevant technique group leader and taken directly to the concierge who will re-program the card as required. If access is to be extended to after hours, the replacement form must be forwarded to HPMRI management for sign off, according to the procedure for obtaining a new card.

Please note Perkins access cards should only be used by the authorised CMCA user. Access to the registered user will be withdrawn if the card is found to have been used by anyone else.

You will be charged $30 for a replacement card.
